Introductions

Sahulat Program is a health insurance initiative of the Federal Govt of Pakistan in partnership with the provincial Govt.

Sehat Insaf Card are to improve the health status of the population, especially the poor, and to reduce poverty through a reduction in out-of-pocket (OOP) expenditure on health care.

Under this program, those having a Sehat Insaf Card are entitled to free health services in case they are admitted to a hospital. Currently, outpatient (OPD) services are not covered.

Every provincial government is managing their Sehat Sahulat program and Sehat Insaf Card under their own management structures. For detailed information about the provincial programmed, please follow the links provided on this page.

نیا پاکستان صحت کارڈ کے تحت ہسپتال میں داخلے کا طریقہ کار

٭             اس پروگرام کے تحت آپ کو منتخب سرکاری و نجی ہسپتال میں داخلے کی صورت میں نیا پاکستان صحت کار ڈ کے تحت مفت علاج کی سہولت فراہم کی جائے گی۔

٭             ہسپتال میں داخلے کیلئے جانے سے قبل یہ اطمینان کرلیں کہ آپ کے پا س نیا پاکستان صحت کارڈ، آپ کا اصل قومی شناختی کارڈ اور بچوں کے    داخلے کی صورت میں ان کا  ”ب فارم“  لازمی موجود ہو۔

٭             ان دستاویزات کے ساتھ ہمارے منتخب کردہ ہسپتالوں میں جن کی فہرست PHIMC کی ویب سائٹ پر موجودہے،  میں سے کسی بھی قریبی ہسپتال کا انتخاب کریں۔ وہاں پہنچنے پر اس پروگرام کا نمائندہ ہسپتال کے استقبالئے پر آپ کی رہنمائی کیلئے موجود ہوگا۔

٭             نمائندہ آپ کے قومی شناختی کارڈ کی جانچ کرے گا اور متعلقہ شعبے تک آپ کی رہنمائی کرے گا۔

نیا پاکستان صحت کارڈ سے مستفید ہونے کیلئے اپنا قومی شناختی کارڈ جلد بنوائیں اور اپنے تمام کوائف (مثلاً شادی، پیدائش، وفات وغیرہ) کے نادرا میں جلد از جلددرست اندراج کو یقینی بنائیں۔

How To Use Sehat Sehat Insaf Card / Qaumi Sehat Card

STEP 1:
Check your Eligibility
SMS your National Identity Card number to 8500, to check your eligibility in the program.

STEP 2:
Get your Sehat Insaf card / Qaumi Sehat Card
In case you have been declared eligible, you can receive your Sehat Insaf card / Qaumi Sehat Card from the card distribution center developed in your district.

STEP3:
Gather Information and Documents required
Determine the empaneled hospitals for Sehat Sahulat Program.
Take the following documents when you visit the impaneled Government/ Private hospital.
Sehat Insaf card / Qaumi Sehat Card
Original CNIC
B-Form (In case of child treatment)


STEP 4:
Use your Sehat Insaf card / Qaumi Sehat Card to get treatment
After reaching the empaneled hospital you may approach the dedicated SSP representative counter for further assistance. The SSP staff shall verify your Sehat Insaf card / Qaumi Sehat Card and will guide you to the relevant hospital department for the treatment.

STEP 5:
Get the treatment free of cost
The cost of treatment after the patient has been admitted to the hospital shall be charged from the Sehat Insaf card / Qaumi Sehat Card.
NOTE: This facility is only available in case of patient needs admission to the hospital

Provide Feedback
Call us at 0800-09009, for your feedback. You will also receive a call from the SSP staff to take your feedback for the experience and treatment received.

All residents of Khyber Pakhtunkhwa are already enrolled in KP's Sehat Card Plus Programme.

This interface is provided by the Ministry of National Health Services, Regulations, and Coordination, Government of Pakistan.

You can also SMS your National Identity Card Number to 8500 to check your eligibility.
